Hart2Hart does not currently have an adoption fee at this time, however we ask that a donation is made to our rescue. The average donation that we recieve for mixed breeds are anywhere from $40.00-$60.00 and $100.00-$200.00 for pure bred animals.

I____________________________________ HEREBY AGREE TO THE FOLLOWING TERMS AND CONDITIONS PERTAINING TO THE ADOPTED BY ME FROM, HART2HART(Breed_________________________/Color___________________ Sex_____) IN CONSIDERATION OF HART2HART'S AGREEMENT TO PROVIDE THE UNDERSIGNED WITH THE PET IDENTIFIED HEREIN:
______ I understand that I am solely adopting this animal and in no event shall this animal be resold to any person, broker, breeder, medical institution, research institution, or used solely as a guard animal, and that in the event that I must ever give up this pet for any reason whatsoever, I shall notify HART2HART and return it to them upon request.
______ That this pet will be spayed/neutered, if it is younger than breeding age, it shall be altered by the time it is capable of breeding and a certificate of sterility will be presented to HART2HART upon request.
______ That I shall provide the proper food, shelter and medical care for my adopted pet, In the event that I can no longer provide the necessities for this pet, I will contact HART2HART.
______ That I shall provide identification tags and all necessary immunization shots and Rabies vaccination for my pet.
______ I agree that if living within the city, all municipal ordinances will be followed.
______ I agree that HART2HART may, at a mutually convenient time, make a follow-up visit.
______ Failure to fulfill any of the above terms will constitute a breach of this agreement and will authorize HART2HART to reclaim this pet and/or take legal recourse.
______ All animals placed by HART2HART are, to our knowledge, in good health. However, we recommend that you have your new pet examined by your veterinarian immediately. HART2HART is not responsible for ongoing medical expenses.
