HOUSE INFO:
Is this animal a house pet? Yes
Is the animal house-trained? Yes – but will pee/poo inside if anxious and left alone
Does the animal ever have house-training issues? On occasion, it’s situational. Never used to, but since daughter has moved out she is struggling
How do you handle house-training errors? Bought a kennel- no longer an issue
Is the animal crate trained? Yes, we had not been using the crate and have begun again.
How is the pet contained if not crated? Leash, or tied in the backyard
Does the animal chew or scratch household items? No
Does it get into the garbage? Rarely, again if left alone for too long uncrated or has access

YARD INFO:
Does the animal dig holes? No
Does it try and get over or under fences? No
Is the pet protective of fence space? No
Is the pet an excessive barker ? or just alerting to things? Just alerts to people mostly

How does the pet react to kids at a distance/ on leash? Reaction? Wants to play/run
Exposure to strangers? reaction on walks or at the door/ in home ? Reaction uncertain shy, barks but is skittish with big movements of strangers. Better with women than men, but warms up with time and patience
Is it good with cats? reaction?- on/off leash ? Reaction? Lived w cat before, no issues.
Is it good with dogs? sizes/ reaction?- on/ off leash? Reaction? Lived with dogs before, no issues.
Is this dog used to sharing its space / toys with other pets/ dogs if visiting etc? Reaction ? Yes, although we have again begun using a kennel so she has her own space.

OBEDIENCE / TRAINING :
Does the dog play games? Fetch, running
Has this pet had any obedience training? No
What commands does it know? Very smart. Sit, lay down, come, go, wait, be good
Does it walk well on leash? Or pull etc? She definitely pulls, would do well with a strong hand. Using a Gentle Leader/ Halti would greatly help as well as more training/ exposure so she learns what is wanted
What walking tools do you use to walk your dog -{ flat collar, gentle leader, martindale, choke collar, Pinch collar, Harness } she is not waled often so has little experience. We have tried a flat collar, no difference in her pulling. We were advised to use a gentle leader.

PERSONALITY/ BEHAVIOURS/ :
** Please be detailed in these questions. No Blanket answers of “Yes/ Sometimes” .We need detailed info to know how best if rescue can help and what the pets needs are for re-training etc .Without this we can not consider , possibly approve or place any dog into foster care” **

Would you say your pet is a stable, well mannered, pet ? Yes, she is very smart and is eager to please. She needs a “person” – someone she can be with most of the time. This is what my daughter was to her and she is missing that – my other children are too young and I don’t have time to be that person for Luna.

Personality/ reaction- with those it knows? Excited, jumps, wags tail, gives kisses, wines, rubs against person.
Personality/ reaction- with strangers ? Barks, approaches cautiously, shys away, if given time will get closer and allow pettings and eventual calm and acceptance.
Has the animal ever shown/ possession/ aggression/ reactivity to anyone or anything? No, she is more likely to run away than be aggressive. Her nickname is chicken because she scares easily and can be very anxious with noises she is unfamiliar with.
Has the animal ever bitten in the past? No.

Are there any behavioral problems? If yes what are they? Since my oldest daughter has moved out, Luna seems more withdrawn/depressed and is struggling more – she has started having accidents in the house, We have started keeping the doors closed and bought her a kennel and the issues have stopped. She is always looking for her “person” and when my daughter comes home to visit, Luna loses her mind and cries for an hour when she gets here. When she leaves, Luna seems to slip back into sleep all the time mode.
Is there any separation issues? Yes. As explained above. The kennel is helping.
Are there any fears? Loud noises, beeping (like fire alarms in the house, outdoor noises don’t seem to bother her as much), thunderstorms. She gets very anxious and pants and paces.
Are there any possession issues with food/ toys/ space towards people/ kids or other pets? No, she has never had any issues with these things.

ROUTINE:
Where is the animal kept during the day? I work from home, so she is free to roam around the house in the daytime.
Where is it kept at night? She used to sleep with my oldest but is now kenneled at night.
How long is this animal generally left alone in a day? A few hours at most, if I have to go to the city or am visiting friends in the day.
Is it ever kept outside, and if so, for how long? She is not usually left outside longer than ½ hour.
Is it used to being chained or kenneled or running free? Outside she is chained/ on a tie out , as we don’t have a fence, but in our last home she ran free in the fenced yard.
Have you ever boarded the animal or had a pet sitter out of your home environment? We have a pet sitter but it is a friend of my sons so Luna knows him.
What is the animal being fed? Pedigree – she is fussy with food.
Is it free fed? Yes, we fill her bowl in the morning, and again at dinner if she ate the morning food.
How much per day? 2 bowls/day
Is it used to bottled water? No, filtered/tap water.
Please provide this pets current routine:
Get up, go outside, pee, drink/eat, sleep, go outside pee/poo, drink, sleep, watch me cook dinner, play in the yard/walk, drink/eat, go pee/poo, sleep. Repeat.

The best home for this pet would be:
Luna would do best with someone who will be with her quite a bit. She LOVES her person and people she knows !! She is good in a kennel and can be left alone for periods of time but would really do better with someone who can spend a lot of time with her. A existing dog in the home would help her anxiety also .
She is very smart and loves to play and be outside. She is affectionate and wants to be with her person. She would do well with a big yard to run and play. We know that there will be a transition period and we are all she’s ever known, but there comes a time when you must acknowledge that this is the best thing for Luna. With my work and the kid’s activities, she is just not getting the attention that she deserves and it is showing in her behavior.
Whatever we can do to make this adoption easier, we are willing to try.
